Error en formulario!!!

Hola, la pagina de contact no me rula, y quiero exponerles un error, cuando voy a modificar mis preferencias en el formulario, la seccion cambiar E-Mail esta errada, dice antiguo PASSWORD y nuevo PASSWORD en lugar de EMAIL.
